The form templates are available here so that the requestor may become familiar with the information which will be requested on web request forms. The templates are not editable and cannot be used to submit a request. You may submit a request from our list of Currently Available Collections.
Request for Materials (Readonly)
If you are interested in requesting specific collection materials from NIA, update and submit the form below and a member of the AgingResearchBiobank staff will contact you within 7-10 days. If desired, you may save the form instead and return to complete it at a later time. Note that the request form will timeout after one hour of inactivity. To ensure no input is lost, please save within one hour of beginning the form. For further details regarding the request workflow, please refer to the User's Guide (PDF - 1,678 KB).
For a Letter of Availability, please complete all required fields to the best of your ability at the present time. Information that is only applicable after availability is known can be indicated in the field as appropriate. Please allow up to six weeks for the Biobank to provide a letter.
You will need to complete the required fields below and provide the following documentation before your request is reviewed by the Biobank Scientific Review Committee (BSRC):

If you do not have funding and require a letter of availability for a grant proposal, please note this in the "Support Information" section below. Note that the repository generally prepares reports within six weeks of receiving criteria applicable to the requested studies.
Once you have funding available please contact us through the Comments section of your request to continue with your request.
If you already have funding, you will need to complete the required fields below and provide the following documentation before your request is reviewed by the Biobank Scientific Review Committee (BSRC):
- Protocol (See Glossary for more information)
- IRB Approval (or waiver)
- CV
- Proof of Funding (Biospecimen Requests Only)
The BSRC will only review a request that has submitted all of the required documentation, noted above. The documents submitted will be examined to ensure they comply with NIA requirements prior to the BSRC's review. If documents are missing your submitted request will not be reviewed by the BSRC until the documentation is complete.
If your request is approved, the Outgoing Human Resources Transfer Agreement (OHRTA) will need to be signed by the PI and the institution's signing official.
